Questionnaire for Kindergarten Parents

St. Pascal Baylon Catholic School is now accepting applications for the 2012-13. Please contact Principal Martha Laurent at 651-776-0092, or e-mail martha.laurent@stpascals.org for more information or to schedule a tour of our school.

To enroll a new student in Kindergarten we require:

1. A New Student Registration Form.
2. A completed Tuition Contract.
3. A completed SMART Tuition Management Form, with a voided check or deposit ticket from a checking or savings account attached.
4. A completed Questionnaire for Kindergarten Parents.
5. A copy of your child's Pre-school Screening results.
6. A $120.00 per family non-refundable registration fee.
7. A copy of your child's birth certificate.

To enroll a new student in Grades 1-8 we require:

1. A New Student Registration Form.
2. A completed Tuition Contract.
3. A completed SMART Tuition Management Form, with a voided check or deposit ticket from a checking or savings account attached.
4. A completed New Student Information Sheet.
5. A $120.00 per family non-refundable registration fee.
6. A copy of your child's birth certificate.

If all classes in your child's grade are full, your $120.00 deposit will ensure your place on the waiting list for your child.

If a waiting list is necessary, we will accept children for admission in the following order:

1. A child with siblings already enrolled at St. Pascal's School.
2. A child of parishioners of the Church of St. Pascal Baylon.
3. A child of parishioners at another Catholic church.
4. A child who is non-Catholic.
